custom reply to settings not respected

      Description

      the civimail custom reply to settings are not respected. regardless of what you set, the field is visible when constructing an email.

      i did some comparison to see where this option was broken. the code is different in v3.4 and v4.0 with regard to this file. i think there was a 3.4 commit that wasn't copied to the 4.0 branch.
