Pledge amount field missing on pledge add page

            Description

            Saw this issue on the demo site. Go to this page:
            http://drupal6.demo.civicrm.org/civicrm/contact/view/pledge?reset=1&action=add&cid=43&context=pledge

            The pledge amount field does not appear. Then if you save the validation fails.

            Also applies to the pledge edit page as well.
