Add Date and Amount of last contribution to Survey Detail

When conducting surveys in the field, it's useful to have the date and amount of the contact's last contribution. These fields should be added as optional Display Columns in the Survey Detail Report.

We have note suggesting that Eileen might have code that provides all or part of this functionality.

[Comment DG & DL]: We're thinking that the specific requirements for what contribution data to added to this report will vary from organization to organization. Therefore, it make sense to generalize this project a bit so the goal is to make it easier to add display columns (and filters) to a report. Lobo's thought is that a hook or hooks for reports could be implemented. Then this specific requirement could be added to the CiviEngage module.
