Inline editing breaks after creating first address or phone number

Scenario A:

1. Begin with a contact record that has no street addresses
2. Click "add address"
3. Type in a street address
4. Save
5. Observe - there is no way add another street address (i.e. "add address" is missing)
6. Reload the page
7. Observe - the "add address" button appears again

Scenario B:

1. Begin a contact record that has no phone numbers
2. Click "add phone"
3. Enter a phone number
4. Save
5. Observe - there is no "add or edit phone" link when mousing over
6. Reload the page
7. Observe - the "add or edit phone" link appears

All observations where made in Firefox 13.0.1 under Mac OS X.
