When recording survey response that's defined as a checkbox, response does not get saved in CiviCRM

      Description

      When creating surveys (Survey, Walklist, or Phonebank) and you create a custom field in the Activity of type Survey, or Walklist, or Phonebank to be used as a survey response that has values created as checkboxes - when you record responses and check the value(s) you want for the response, the values do not get saved in the activity record of the respondent. I've been able to recreate the problem in the civicrm demo site.

      Multi-selects, select and text fields work fine.
