Begin generalizing export mapping form to integrate into any pageflow

This isn't a complete implementation, unfortunately. I found it convenient to include CRM_Export_Form_Map from a custom search results export task, and needed to generalize form ids and data. The mapping fields are now displayed correctly in my new pageflow, however fields cannot be added or modified.

If this would be a useful feature for others, I can continue with development.
