      Description

This issue occurs when submitting a credit card contribution in the back office. In this case my payment processor is Authorize.net. Since there is no payment processor in the public demo, I cannot reproduce it there.

To reproduce the issue in my local environment:
      1) Bring up an existing contact record.
      2) Click the "Contributions" tab
      3) Click the button "Submit Credit Card Contribution"
      4) Fill in any amount, any contribution type. Fill in the credit card number, expiration date, security code, and billing address on the card.
NOTE: The first problem: The billing address fields are empty, even though these were pre-filled under version 4.1.5
      5) Click "save"

      6) Click the button "Submit Credit Card Contribution"
      7) Fill in any amount, any contribution type. Fill in the credit card number, expiration date, security code, and billing address on the card.
NOTE: The second problem: The billing address fields are empty, even though these were pre-filled under version 4.1.5 when submitting card for the second time on an existing contact.
