Upgrade contribution_type table to financial_type and financial_account tables

1. Create default financial_accounts as per tarball for all accounts other than income/revenue accounts.

For each contribution_type:
2. create a financial_account and financial_type record with same label/name, ft of type Income/revenue, and an entity_financial_type linking them with relationship of income/revenue
3. add entity_financial_type linking financial_type with each of the default financial_accounts created in 1 using appropriate relationship type.
