            Create custom date field used for an activity

            When the Activity email is sent out (triggered by assigning Activity to someone) the Date provided shows as eg

            Verification Date January 0th, 0 12:20 AM

            The in-built Activity 'Date' field sends out correctly

            eg Date and Time June 11th, 2012 12:31 PM
