Online contribution page WITHOUT membership section should NOT include "No thank you" option

This regression snuck in when doing the fixes last week for membership section WITH contribution section. If only the contribution section is enabled, we should not be inserted the "No thank you" radio option because the user MUST select a contribution radio button OR select Other Amount is that is included.
