Add index to sort_name / is_deleted to civicrm_contact

The default quicksearch and empty basic search generate a query which uses

is_deleted = 0 ORDER BY sort_name

mysql needs an index on is_deleted and sort_name to avoid doing a filesort. Adding this index should optimize this path
