Fix basic formatting problems

Am fixing trailing whitespaces as I am seeing a lot of commits cluttered by whitespace commits which makes them hard to read. PR covers

• replacing Windows EOL with Unix EOL (with the exception of around CiviCase xml & I think Joomla! files as the usage of Windows EOL is prevalent enough to make me wonder if the xml files were done that way on purpose
• removing trailing white spaces (in php exhaustively & in tpl, sql, hlp, xml to some extent)
• replacing tabs with double spaces (notable exceptions include the joomla! dir for the reason above)
• removing old svn //$ids$
• adding licensing block to the top of files

The above is exhaustive on the CRM directory (with the exception of some CiviCase stuff) and through significant parts of tests, templates, sql dir. Other repos not touched. Joomla! dir mostly avoided as inconsistencies are so consistent.
