Create PCP link is hard to discover and link in user guide is incomplete

      Description

      If admins want to invite users to create a Personal Campaign Page, the link format isn't easy to find, and the format displayed in the user guide is incomplete (causes a fatal error):

      1. fix documentation in book for the 4.3 book version
      2. add link format to help text on Personal Campaign Page admin page (civicrm/admin/pcp?reset=1)

      Testing this on:
      http://drupal.demo.civicrm.org/civicrm/contribute/campaign?action=add&reset=1&pageId=6
      and
      http://wordpress.demo.civicrm.org/?page=CiviCRM&q=civicrm/contribute/campaign&action=add&reset=1&pageId=4
      ... and on our local site.

      The error message:
      Supporter profile is not set for this Personal Campaign Page or the profile is disabled. Please contact the site administrator if you need assistance.

      All I did was create a contribution page, enabled one payment processor, set two donation amounts, enable Personal Campaign pages, and provide an email notification address.

      Also, the "Manage campaign pages" LINKS feature should include a link to the links for people to create their Personal Campaign pages; right now the only way to get that link is to look inside the book: http://book.civicrm.org/user/current/contributions/personal-campaign-pages/
