I was able to recreate the problem as requested on http://demo.civicrm.org/.
Having trouble with the formating of custom fields in the form of a radio button. CivicRM provides the ability to specifiy how many options to display per line however, this qualifier doesn't appear to work. Currently no matter what I put in the value, it shows every option linearly instead of putting a <br> after each option. I want to show one option per line.
On the demo site the name of the custom data group is "donation types" and I have it currently connected to "Donation Form":
