Modification of CiviEvent to allow payment fields to appear on the confirmation page

As previously discussed we've modified CiviEvent to allow the payment fields to be placed at either the beginning or the end of the registration process.

I've attached a document describing the files and changes.

Our client used this code to register almost 4700 participants since Aug 1, 2013, including groups of as many as 30 participants.

I'll also submit a pull request against this issue.

Just recall the code was implemented in 4.2 so some tweaks may be required.
