            Description

            Fix bugs in Activity Form
            ====================

            a)
            1. Go to Standalone Activity Form
            2. Choose Target Contact leaving Activity Type empty
            3. Form rule is triggered with messed up Target Contact showed as 'undefined'

            b)
            1. Go to Standalone Activity Form
            2. Choose Source contact A in the drop-down
            3. Change the Source Contact to contact B
            4. It throws errors with missing source contact
