CRM-13733 Survey Question Editor does not show read only fields that are in the profile

      Description

      tested on joomla.demo.civicrm.org.
      to replicate:
      Make a survey. - complete Main Information tab
      go to Questions tab
      create a new contact profile with
      Primary Phone. Mark it as read only.
      Home Phone. do not change defaults
      save the new profile.
      returns you to Contact questions box
      The primary phone field does not appear.
      go to Results tab and select results. save the survey.
      Reserve respondents for your survey.
      Interview the respondents - the primary phone number will appear as read only which is the desired behavior.
      The problem behavior is that it does not show up in the survey set up routine.
      (it also does not show up when you "preview" the profile via the survey profile edit routine).
