Improve Use of honor role for Personal campaign pages

When you are making a donation via a personal campaign page in which the honor role check box is not checked, you are still given the option to have your name listed in the honor role. This check box should not be visible if the PCP does not have honor role checked.

In addition, the default is to not have your donation listed. The default should be to have your donation listed (but anonymously) with the option to change to have your name included. That both preserves privacy and maximizes the outreach opportunity for having donations scroll by in the honor role.
