Refactor and simplify relationship form

            Description

            This is the last form to use the deprecated NewContact.tpl it is also not compatible with the new popups UI.
            Refactoring it to use an entityRef field rather than autocomplete/search combo will solve these as well as several long-standing UX problems with the form, namely:

            • "Quick Save" vs regular save is confusing.
            • The search/select/save is unnecessarily complicated.
            • Creating relationships with multiple contacts is limited to those who all match the same search string.
