CRM/Report/DAO/ReportInstance schema file name is inconsistent

            Description

            Could not replicate the reported missing DAO file. However, we will rename Instance.xml to ReportInstance.xml for consistency.

            The GenCode.php script generates all the DAO classes (amongst others).

            There is a mismatch in the metadata used by this script that causes the CRM/Report/DAO/ReportInstance.php file to not be generated at all.
