Fix pre-4.5 membership line_item entity_table and entity_id values

            Description

            Line item records created for memberships prior to 4.5 use the associated contribution as the entity_table/entity_id value. As of 4.5, these are referencing the associated membership record (and the contribution is referenced in line_item.contribution_id). Records created prior to 4.5 should be updated for the new 'scheme'.
