Pop-up editing on "New Survey" Activity Type is wrong

            Description

            Unrelated to my last bug - the pop-up edit button for "Activity Type" on the "New Survey" screen pops up the general activity editing screen - it should pop up the "Survey Type Options" screen. An easy fix, I'll submit a PR in a minute.
