FK failure in 4.5 upgrade due to hard-coded location_type_id

            Also noticed the the Honoree Individual profile was NOT available to be selected under Manage Contribution pages -> Title and Settings due to an extraneous space in group_type:

            "Individual, Contact" causes it to be excluded from SELECT profile dropdown
            "Individual,Contact" works as expected.

            Added update statement to 4.5.2..mysql.tpl
