Insert form into page dialog button does nothing

When trying to insert a form into a Wordpress page the 'Insert form' button on the dialog box does nothing and the Cancel button is grayed out. So it's not possible to insert any of the Civicrm front end functions into a page. All I can do is click the X in the upper right corner and exit the dialog box.
