            Description

            Add a Date custom field to a record (Contribution, Membership, Grant, etc).
            Find and edit an existing record.
            Notice error in console.log:
            Uncaught TypeError: Cannot read property 'indexOf' of undefined

            All JavaScript processing stops for the page, including client-side form validation.
