Importing Contacts with Notes from CSV causes duplicate note to be created.

            Description

            We are running CiviCRM 4.5.4 on top of Drupal.

            Steps to reproduce when importing from a CSV file:

            Menu
            Contacts \ Import Contacts
            • Choose File - selected the CSV file
            • Checked first row contains headers
            • For duplicate Contacts checked update
            • Continue
            • Continue
            • Import Contacts (1 row) - Import Now
            • Are you sure - yes

            Record gets updated but contains the note duplicated.
