Scheduled reminders: allow both "Limit to" and "also include" within one reminder

The current form only allows for either further restrictions on the original criteria to limit who receives a scheduled reminder OR for it to be sent to an additional group of people who don't meet the original criteria,

There will be times when people want to do both.

I am proposing that we have two sets of "tick box plus two (initially hidden?) fields" covering these options rather than the one set of three fields that exist at the moment.

So instead of the "Limit or Add Recipients" field where you can select "neither", "limit to" or "also include" there would be a tick box labelled, say, "Further limit recipients to" and another one labelled "Also send to".

Ticking the "Further limit recipients to" box would result in the next two fields appearing for that option.

Ticking the "Also send to" box would result in the next two fields appearing for that option.

This would have the added advantage of easily leaving out the Participant role from the options offered for in the "Also send to" that Joe Murray has commented on in https://issues.civicrm.org/jira/browse/CRM-15536.
