empty display en sort_name_format get's filled in with default values

When you set the display_name_format and sort_name_format to be empty, the settings forms fill's those in with the default values.
So everytime you edit the form settings display preferences form, you ahve to explicitly re-empty those values, otherwise the set empty value get's overridden.
