Reports with more than one "Display Options" checkbox don't render correctly

            Description

            In extending CRM_Report_Form_Event_ParticipantListing, I came across a funny bug in which checkboxes on the "Display Options" tab are displayed multiple times. It seems the bug only occurs if more than one checkbox is defined, a case which does not occur in any core report.
