Translations Don't Match for Menu and Page Heading

We are trying to completely translate on our our sites to be fully mutlilingual, and are running into an issue. Our translator has finished the transifex project, but unfortunately the string in the menu bar (for New Individual for example) is not matching that same set of words when it appears on the 'New Individual' creation page. In other words, it appears that 'New Individual' is being pulled from multiple places. We are unable to find where this second usage is being pulled from, and would like the two uses of the phrase to sync in the Ukrainian language. Any answers on this issue are greatly appreciated.
