Free HTML fields do not display in a profile when that profile is used in a Survey

            Description

            Problem description:
            1. Added a field Formatting > Free HTML to a profile.
            2. Used that profile in a Survey (Under Questions, tested for either Contact Info or Questions field).
            3. The Free HTML field doesn't appear on Campaigns > Contact Survey > Interview Respondents screen (civicrm/campaign/vote?reset=1) nor on Campaigns > Interview Respondents > (search, select contacts, Record Survey Responses) > Record Survey Responses (civicrm/survey/search?_qf_Interview_display=true) screen.

            The same profile, when used on a Contribution Page, displays the Free HTML field without any issues.

            I was able to repeat the issue on the demo site.

            Suspecting that the issue may be related to https://issues.civicrm.org/jira/browse/CRM-11955
