Error returned when Role assignments are reused between cases

      Description

      When a contact has multiple cases and the role assignments are the same on the cases, an error is returned when assigning the roles on the subsequent cases. This started to occur when we upgraded our site from 4.5.4 to 4.6.4. This issue has been noted in Stack Exchange and attached for reference. The attached document contains the recreation this issue on the Demo site.
