Fix search form layout for date range fields

Currently, when user selects 'Choose Date Range' in a search form, the From and To date fields are displayed quite a ways off to the right side of the form. This is confusing to many users.

This change places the From and To fields below the date range select (thanks to Adam Wight and Eileen McNaughton for the suggestion / patch).
