Premium is not Saved to DB for Online Membership Signup Case

            Description

            Case: you configure an online contribution page to include a Membership block AND one or more Premiums in a Premium block.
            Bug: If contributor selects a memberhship type and a premium and makes the contribution - the contribution_premium record is not saved to the DB. It IS shown on the Confirmaiton and Thank-you pages. But it also is NOT included in the emailed receipt.

            You can recreate on demo using contrib page 2 (I've added a premium choice to that Membership Signup Page):
