Export Case Activity Fields - missing delimiters

            Description

            Issue began with upgrade to 4.7.1 and can be replicated on demo server, currently running 4.7.4.

            Exporting case reports with Case Activity fields results in a formatting issue. It appears that these fields are missing the comma delimiter as well as missing the column header for those columns.
