CRM-1828 options_per_line in preview does not work

            Description

            When previewing Custom Data fields, each option in a radio or checkbox field appears on its own line, regardless of what options_per_line is set to. Verified in the demo, looking at Constituent Information (Most Important Issue).

            The fix is a small change to template/CRM/Custom/Form/Preview.tpl. Patch attached.
