Description
I've reproduced this on the demo site http://dmaster.demo.civicrm.org with the following steps:
1. Navigate to Administration > CiviMember > Membership Types, then edit the "General" type to set the "Auto-renew Option" setting to "Give option, but not required" (in the current payment processor configuration, enabling auto-renewal required that I also had to adjust the membership period duration to 1 year, but that's not relevant here).
2. Open the "Member Signup and Renewal" contribution page for editing.
3. On the Amounts tab, enable the option "Contribution Amounts section enabled", then define a value ('Additional amount') for "Contribution Amounts Label", and create a couple of amounts under "Fixed Contribution Options". Save changes.
4. On the Memberships tab, under "Membership Types," set the "General" type's "Auto-Renew" column value to "Give option".
5. Now open the contribution page's "Online Contribution (Live)" link, to view the form as a public user would see it. Observe the checkbox labeled " Please renew my membership automatically." is displayed twice: once under the radio buttons for Membership, and once under the radio buttons for 'Additional amount'.
