CRM-19815 Make pseudoconstant code generic in advanced search

I've renamed this ticket so I can close it against the code merged to 4.7.17

I am working on fixing the performance on option value fields in advanced search. However, I will move the actual improvements to a follow on and close this on the basis that it covers the code improvements to allow me to extend the pseudoconstant handling to non-contact searches.

This involves code change & removal around gender_id, prefix_id, suffix_id and communication_preferrences and communication_style, with handling being changed to be more metadata based & mor generic
