Number of Locations (Address Setting) Doesn't Work for Values Greater Than Two

            Description

            Administer CiviCRM >> Global Settings >> Address Settings includes a field to set "Number of Locations" for the contact edit form. However, this isn't working for values greater than 2. (e.g. enter 4 in this field - the Edit form still only includes 2 locations).
