Description
The SurveyDetails report is supposed to be used to help canvassers complete surveys via door to door knocking.
When output'ing to print or PDF, it should create a cover sheet with the full questions (aka field names) printout out.
Then, on each page, it should only have Q1, Q2, Q3, etc in the header to preserve space.
However, now, some field names are being printed and some are being converted to Q1, Q2, etc.
