Additional locations created automatically from create new contact form

When max location number is greater than 1, the form for creating a new contact seems to automatically create the additional addresses. The only way to delete is to submit the contact form and then edit to delete the additional locations.

Moreover, if the first address's location type = "Main" (or whatever the secondary default would be in the next location) the form returns a validation error, "Two locations cannot have same location type" even though all other fields (except the pull down menu for location type) in the 2nd & 3rd locations are empty.

Creating a new contact with 1 address but where max locations = 3 results in 3 corresponding rows in civicrm_location > civicrm_address where 2 addresses are NULL but with corresponding location ids.
