Custom fields of type select not displaying in Activity Report

            Custom fields of type 'Select' are not displayed in the activity report. The column is there but the entries are blank.

            The data in the field should appear in this column.

            Description

            I've spotted a regression in the Activity Report in CiviCRM version 4.6.32.

            If you have a custom field of type 'Select' the data isn't displayed in the report.

            I have recreated this on the 4.6.32 Sandbox site http://d46.demo.civicrm.org/civicrm/report/instance/3?reset=1

            When I run this report, I get the following error:

            • Notice: Undefined offset: 2 in CRM_Report_Form->formatCustomValues() (line 2061 of /srv/buildkit/build/d46/sites/all/modules/civicrm/CRM/Report/Form.php).
