Mailing Header and Mailing Footer are NOT Included in Mailings

If a Mailing Header (component) and/or a Mailing Footer are included in the mailing content - neither of them is being included in either the TEST or LIVE mailings. The "token validator" DOES see that required tokens are included in the footer (using the samples below) - the the content never gets output. Also don't show up on Previews - which ideally they should.
