Amount granted appears in amount requested

The field labels on the form weren't matching the listing selector and where a bit confusing. I've updated these to make them consistent and tested that form value are showing up in correct columns in listing.

I created a test grant to myself. The total amount granted was $56. When looking at the summary of grants the amount granted was listed as $0 and the amount requested was $56.
