Offline Membership Signup and Offline Event Registration WITH Payment Recorded: Include Membership Type or Event Name in Source of Contribution Record and Include Transaction ID Field

1. When doing offline event registration WITH the "Record Payment" option - the resulting contribution record should have the Event Title prepended to the value in the contribution source field. *** This part has already been coded for 2.0 - but no issue was filed, so noting it here. ***

2. When doing offline membership signup or renewal WITH the "Record Payment" option - the resulting contribution record should have the Membership Type prepended to the value in the contribution source field. (this needs to be done for 2.1)
