CRM-313 Custom Data Field?s Default Value availability inconsistency

      Description

      1. Go to a new Custom Data Field design page (in Administer CiviCRM)
      2. The default is Alphanumeric/Text ? change it to Alphanumeric/Select (the Multiple Choice Options appear, the Default Value disappears)
      3. Change it to Yes or No/Radio (the Multiple Choice Options disappear, nothing new appears)
      4. Change it to Alphanumeric/Text (the Default Value appears)
      5. Change it to Yes or No/Radio (the Default Value does not disappear)

      The avaliability of the Default Value field should be more consistent.

          The same happens if you substitute Note/TextArea for Yes or No/Radio.

          The same happens if you change Yes or No to Note, Date, State/Province, Country (i.e., any field type not containing 'Text' sub-type).

          Fix applied to javascript which shows/hides default form field. Also moved javascript from .php to .tpl file. rev2659.

          The Default Value field now appears as expected. As for comments of its behaviour, see CRM-319 (this applies here, too) and CRM-320.
