UI Improvement: "Use for Bulk Mailings" should be a radio, not a checkbox

            Description

            Related to CRM-3523.

            Since CiviMail sends to only 1 email address per contact ( http://wiki.civicrm.org/confluence/display/CRMDOC/Mail+Generator ), having a checkbox is misleading. Radios would be better. And since this rule spans multiple locations/emails the radios should reflect this. i.e. Clicking a radio should clear radios for all email addresses for all locations.
