CRM-3666 install script gives error when civicrm db non-existant

If the civicrm database has not been created before running the install script then the user receives the error that the mysql user id specified does not have the 'create temporary table' permission. The documentation says that the script will create the database using the supplied name. The work-around is to create an empty database and then run the install script.
