CiviMail should have the option of not sending duplicate emails

Different contacts ids could potentially share the same email address. The user may want to suppress sending multiple copies of the mailing to one email address (even though tokenization might result in slightly different email messages).

We will add the option to dedupe by email to the first form step (groups), and store the value in the db.
