When a profile is used as a sign up form and you have "update contact on duplicate match" checked and you have recaptcha enabled for it, when a duplicate is found, it gives an error saying "an account already exists." Despite the fact that profile is set to update duplicates.
When you disable the captcha but keep update, it works. I was able to replicate on the CiviCRM demo and this is on a 2.1.4 database.
